2009 running lean

2009 that is running lean. I get a smell from inside the cabin while driving. Also I can see soot on the exhaust tips. I get a P0420 code on and off. I changed the bank 2 O2 sensor after cat. Also had spark plugs changed and valves cleaned. But I still have this smell and MPG says it is 15.6 but I don't think that is right. I'm assuming it is a vacuum leak of some sort. I'll be pulling the smoke machine out I just ordered but was hoping for anyone's advice who went through this already.

Did you use a new intake plenum gasket when you changed the plugs and put everything back together? If not, that could be the source of any vacuum leak.

I took it to a garage due to a flashing engine light. They said they replaced spark plugs, gaskets and cleaned valves. It had a P0420 code before hand.

Maybe "they" missed something. Good luck. Does the shop have any kind of workmanship guarantee? It's difficult giving advice after someone else has worked on a car. Check all the vacuum hoses and fittings before you do anything else. There's an unused vacuum nipple on the intake near cyl. 1 which sometimes folks don't plug properly. You'll have to remove the top cover to find it.

I believe the nipple was removed from the intake once the engines became DI.
The nipple was there on 2008's. And possibly 2007s.
